  • Family Call - We know family is important. That's why we'll call you within 72 hours of your loved one's move-in to report on their status. We also call 45 days after admission and then every 30-60 days prior to Customer Satisfaction Survey.
  • LifeShare - Stay connected with those you love. The LifeShare mobile app will allow you and your family members to send pictures and text messages, and also keep up-to-date on campus activities and events.
  • Inspired Living Recreational Programs - These are a variety of events, activities and outings available for you to choose from. For a list of all available programs, please see your LifeShare monitor or speak to a member of our Life Enrichment team.
  • Happy Hour - Every Friday, residents and their family members can participate in a happy hour where they can mix and mingle while enjoying beverages and snacks.
  • Restaurant-Style Dining - This includes an open breakfast from 7:00 to 9:00 a.m., our Cafe Menu Alternatives, Salad Bar, Hand-scooped ice cream, Homemade soups and fresh fruit baskets.
  • Sunday Brunch - Enjoy Sunday brunch with us once a month. This includes complimentary meals for two of your guests.
  • Transportation - Transportation is available to and from physician's visits. Additional charges apply.

PAYMENTS

Improved Payment Experience. Fewer Hassles.


With a single enrollment, you can now set up recurring payments using a variety of methods for your Trilogy account including credit card*, debit card* and e-check. Below are the instructions for accessing the new, convenient payment portal and enrolling so you can make secure, recurring automated payments.

*Please Note: If you currently have your payment information set up with our previous vendor, you will need to complete this new set up process with our new vendor by going to the campus website and clicking on the Pay My Bill Link.

  • With your paper statement in hand, click on the Pay My Bill button on your facility's homepage from the website URL as shown on your statement.
  • Click on the Make a Quick Payment button to make a one-time payment by credit card*, debit card* or e-check. Enter required information exactly as it appears on your current monthly billing statement.
  • Click on the Sign Up for My Billing Summary button to set up an online profile account which will allow you to view your statement online with options for setting up automatic monthly payments. Enter required information exactly as it appears on the current monthly billing statement. Be sure to store your login information and password in a safe place. *Please note: The Enrollment User Full Name must be entered without spaces.
  • Once all required fields are completed and the form is submitted, you will receive an email to the email address provided that will include your temporary password.
  • Click on the Link to activate your account using the temporary password received in the enrollment e-mail. You will then be prompted to create your own user password.
  • Once your account has been activated, proceed to the portal to add your desired payment method information under the My Wallet section on the left-hand menu, and then add your desired payment method information. 

*A 2.85% Surcharge Fee will be charged when paying by credit or debit card.

Short-Term Rehab/Home Again


Services available specifically to our Short-Term Rehabilitation residents include:

  • Home Again Personal Wellness Plan - Our staff will provide you and your family ongoing education concerning your diagnosis.
  • Celebrate Success - Your attending and specialty physician will receive up-to-date communication on your progress during and after your stay.
  • Education - Education supporting your wellness now and when you return home is offered weekly.
  • Therapy Home Evaluation - Prior to returning home, your therapy team will complete a home evaluation to determine what you may require in order to be successful at home after you leave rehab.

While at our campus, each resident will enjoy these features:

Upon Discharge:

  • Customer Satisfaction Survey - Follow up with us after your stay, and let us know if you have any comments or suggestions regarding our service.
  • Discharge-to-Home Meal - To ensure you have a nutritious meal upon arriving home, we will send one home with you. Having an easy to fix meal saves you from having to worry about grocery shopping as soon as you return home.
  • PCA Cares Medication-to-Home program - Did you know that one of the leading causes of hospital readmission is failure to take medications as prescribed? As an added convenience, we make going home again easier with our PCA Cares program. Through PCA Cares, we will fill your prescriptions and provide them prior to you leaving our facility. This allows you to have all of the medications you need for the next 30 days without having to stop at your pharmacy on the way home.
  • Senior Executive Club - Become a part of the SEC and participate in monthly meetings with educational presentations, complimentary lunches and fellowship with other seniors.
  • Outpatient Therapy - Enjoy the benefits of having access to our comprehensive therapy team, even if you're no longer a resident.
  • Almost Home Program - If you feel that you need a little extra time to recuperate after your rehab is complete, you can choose to stay on with us for a longer period of time. Ask a member of our team for more information about our Almost Home Program.
  • Volunteer Opportunities - Did you love the time you spent at our campus? It doesn't have to end. There are many volunteer opportunities just waiting for someone like you.
  • Support for a Smooth Return - You'll have priority admission should you need to return to our campuses for service. Additional coverage through your insurance may be available if you returned home too soon.
